If you want to take advantage you need an EFT and at https://openeft.org you can make one. You can either use an existing FD-258 or you can make one with the guide at https://openeft.org/fd-258-guide/, you just need a normal printer and a $3 pigment ink pad.
Start to finish should take you 10 minutes and you can reuse an EFT on any form 1 or 4 for a whole year
